Complete every bracketed field, choose applicable options and remove unused text. Remove the guidance page from the recipient’s copy. Keep the source acknowledgement when sharing the template.
- Follow the organisationÔÇÖs reporting route. If the concern is about the usual recipient, identify an appropriate alternative.
- Keep the account factual, with useful dates and evidence. Informal action is not a prerequisite in every situation.
- Keep a copy. Raising a grievance does not itself stop an employment tribunal time limit; check current guidance promptly where relevant.

What we adapted
Added optional context, evidence, contact and adjustment prompts. The structure remains a request to consider a grievance, not a finding about the events.
