Complete every bracketed field, choose applicable options and remove unused text. Remove the guidance page from the recipient’s copy. Keep the source acknowledgement when sharing the template.
- Record receipt and calculate the decision deadline. Current Acas guidance provides a two-month period, including any appeal, unless an extension is agreed.
- Consult before rejecting a request. An acknowledgement is not an approval or a refusal.
- Keep any disability adjustment request in view as a separate obligation. Recheck current requirements before using the template.

We aim to communicate the decision by [date within the applicable decision period]. If more time is needed, we will discuss and seek agreement to an extension rather than assume one.
